does any body know anything about the specialist pathway.

actually Im a surgeon and I have finished my masters degree and MRCS ENG

if anybody know what is my oppurtunities as I understand that it is very long and expensive pathway.

so Im very confused weather to take the amc exam pathway or to take the specialist pathway.

by the way, Im now in australia doing a trainig related to my MD degree

I would like to ask Dr. Ziad some questions.

1st I will explain my situation for him.

I work as an assistant lecturer of generaland digestive surgery in egypt

I have finished my master degree 2007 and finished MRCS)eng 2008.

I was lucky to have a fully funded grant to finish my MD degree in collaboraion with JCU university in townsville, queensland.

Im now in Townsville , a very small city but Im fine thanx god.

I have surfed the site of the australian royal college of surgeons and the AMC to see my oppurtunities.

at the end I found what is called the Dual pathway that encountries doing the AMC exams and the specialist pathway assessment.

it costed me 275 AUD to appply online.

I sent them my certificates of masters and graduation and CV amd MRCS and specialist registeration in the egyptian medical syndicate and the certificate of good standing and logbook for my residency and 3 reference letters from my senior surgeons in cairo.

and Im still waiting for there reply.

what Im really concerned about is the specialist pathway.

does it worth to go through??

I have contacted an ENT consultant in national cancer insititue in cairo who had md from about 12 years and have alot of publications and experience.

he told me that it took about 4 years for him in this pathway and finnaly they refused him even without an interview.

and that costed him too much money

so comparing my self to him Im just a kid :h!!!h:))

I have only masters MRCS and 6 years experience in general and GIT surgery., and he is a consultant from about 12 years.

so I don't wanna to spend a lot of money and waste alot of time without any benefit.

as just the document assessment cost about 4000 AUD and the interview is about 3000 more.

so it is 7000 AUD and after that they will say what I need.

even I understand that if they say that I have to do a supervisied training it will be upon my expenses.

so will it worth to go through that??

or I just forget this pathway and concenterate on the AMC pathway.

التانى عا,دكتور مصرى سافر السعوديه على شغل حوادث,عمل FRCS بتاعت UK وكمل جراحه وبعدين جه هنا,بقاله 3 سنين بيحاول يدخل ال training بس مش عارف وألسبب إنه مش معاه ولا general registration ولا permenant residancy لأنهم من شروط الجراحه والسنه الى فاتت بس نجح AMC1

أعتقد الفرق بين الأتنين هو الأقامه لأن الأول معاه إقامه دايمه من جوازه من إستراليه

ربنا يكرمك يا دكتور يعنى رأى حضرتك اكمل اللى انا كنت بعملوا ... بس سؤال لحضرتك تانى هو انا هبدأ من السنة ديه مذاكرة ان شاء الله فايه رأى حضرتك فى المصادر اللى اذاكر منها يعنى انا ناوى مثلا اخلص النسا فى الكلية احل عليها من الكتب الاجنبية و كمان اذاكر من كتب تانية للمعلومات زى oxford handbooks مثلا بس محتار اجيب ايه اذاكر منو و اخل فيه فلو تعرف تقترح كتب مهمة و هتنفع للمعادلة دا طبعا بعيدا عن الكتب الاساسية بتعتها anthology & anonnated لان مش هبص فيها غير و انا خلاص بجهز لدخول الامتحان ... ياريت اعرف رأى حضرتك و شكرا جزيلآااا

أسف على التأخير بس حضرتك عندك سنه قبل ما تبدى مذاكره عشان كده أتأخرت فى الرد,أنا ذاكرت من ال Oxfords وحلت MCQ القديمه وبس

ده هو نظام الضرايب هنا والأرقام ديه بتتغير كل سنه بس هشرحهالك مع أنى شايف إنها مش هتفرق حاليا

لو حضرتك بتقبض مثلا 80 ألف

يبقى أول 6 ألاف دولار مش عليهم ضرايب وبعدين بيخدو 15 سنت من كل دولار بين 6 و34 ألف وبعدين ياخدو4200 دولار + 30 سنت عن كل دولار بين 34 و80 ألف وأحسبها بقى حضرتك

كل عام و انتم بخير جميعا و السنة الجاية ان شاء الله كلنا في استراليا مع د/زياد.

بس حبيت أسألك يا د/زياد بالنسبة للاختبار العملي بتكون من ايه ، وكيفية الاستعداد ليه؟

الأمتحان ال Clinical بيكون 18 محطه فيهم 16 حاله والأتنين الفاضيين بيبقوا راحه,تقعد تشرب ميه وتهدى كده,كل محطه ( حاله) سبع دقايق دقيقتين تقرا المطلوب وخمس دقايق تنفذ وتجاوب على أسئلة الممتحن لو فيه,الحالات فى كل حاجه وبأشكال مختلفه يعنى فيه Physical examination مثلا بينطلب فيه مثلا تعمل examination of a diabetic foot أو Neurological examination أو Knee or hand examination أو ممكن يكون counciling مثلا زى مريضه بتشتغل سواق على مقطوره وحصلتلها أول fit وأتشخصت كا epilepsy لازم تجاوب على أسئلتها وهل ينفع تسوق تانى ولا لأ وكده أو مثلا واحده prostitute حامل وأكتشفت أنتا أن عندها HIV ,هتقولها إيه وهتعمل إيه هتكمل حمل ولا لأ,إيه فرصها إن إبنها أو بنتها يبقو عندهم نفس المرض أو تكون حالات emergency ,مريض عنده chest pain مثلا ,أقرا ال ECG وأسأله وعالجه كما لو كنت فى مستشفى أو مريض جايلك بعد Snak Bite هتعمل إيه وتعالجه أزاى

غالبا الأمتحان بيكون خليط منهم كلهم,حاليا الأنتظار للأمتحان العملى حوالى 14 لى 16 بعد AMC MCQ وتمنه 2500 دولار على ما أتذكر والمذاكره بتاعته محتاجه مجموعه و فى كتاب بتاع examination بيذاكر منه طلبة الطب هنا إسمه Tally & O'Connor ( http://www.us.elsevierhealth.com/product.j...=9780729537629) وال AMC عندهم كتاب Clinical فيه حوالى 300 حاله بيجى منهم 5 أو 6 حالات وطبعا إمتحانات السنين الى فاتو

وده مثال للحالات لقيته

1) My first case was a snake bite . How do you manage it? in the ankle, pateint stable, young, bitten 30 minutes back

2) A 3 yr kid with previous dx of asthma, hospitalized for a week , he is going to get discharged, Talk to the father (counselling) Spacer counselling

3) Liver metastaisis, counsel, and tell about the diagnosis and investigations (repeat from adelaide)

4) A patient with known hemaoohoids 65 yrs, comes with left illaic fossa pain pain and discomfort, in further talking we realise that his father has colon cancer, talk to him about his investigation. He was tired, no waight loss, no colonscopy done before. Might be cancer so rule that out

5) Patient with known 1.5 cm breast mass, FNAC diagnosis cancer, patient asked what the specialist will do? Cure rate No lymph node clinically palpable. Discuss about tt

6) young girl with severe headache from yesterday morning, furter photophobia, neck stiffness, ask for CT scan and admit to hospital with dx of subacranoid hemorrhage. Slight fever so rule out meningitis also.

7) 30 hour newbornwith jaundice but parents have noticed jaundice before 24 hours as well : ABO incompatiblity, pt is well but yellow manage and counsel fater. Mother 0 + and baby A+

8) Girl caring for a end stage cancer mother, all of sudden she lost her voice. Funny station. Ask about HEADSSS, she could only nod yes or no. Asked about suicide, and finally I get her to answer me by writting, but the time run out when I had asked just two questions to be written including her name.

9) Examination of diabetic foot. No finding except glove and stocking sensory neuropathy. No motor, no pulse. basically normal person acting as a diabetic.Vibration was ok.

10) young 40 yr od man , first time UTI manage it, Since it is first in male below 60 he sould be investigated. He was asking why should I get investigations. What antibiotics > trimethoprim.

11) Patient has huge pneumothorax, without symptoms, first episode. Young male. Hat to do, when someone else asked about aspiration he was asking are you sure. I just managed conservatively.

12) Recurrent abortion at 8 weeks and 10 weeks andeveryting else including all imaginable investigations were normal. Including blood : anti phospholipids, USG, chromosomal, hisograms. pt asked about cerclage, but I refused as it is not 2nd trimester abortion. Referred to get rid of the case

13) 5 yr old boy, with fever and leg pain, slightly under the knee. without any other problem, Possible osteomyleitis, Xray and DD

14) gestational hypertension : 34 weeks now. 150 / 100. Rcollege of obst says that follow with freq checkings, no drugs , I did that. no proteinuria, no odema.

15) 55 yr old female with incontinence what to do. Investigations ordered and asked to do pelvic excercise.

16) Schizophrenic with olanzapine complaining of weight gain, talk ask and investigate., also ask about diabetis.
