How to approach the MCQ Exam

The AMC provides its own information that is intended to help candidates prepare for the AMC MCQ exam and this is available at http://www.amc.org.au/preparing.asp.

These are notes are intended to complement the AMC’s information.

Why have an MCQ Exam?

The MCQ has been showed to be the most valid and reliable assessment method for assessing knowledge. It is easy to administer and mark, and can cover many different topics.

What is an MCQ exam?

There are different types of MCQ, but the AMC exam currently uses “type A” otherwise known as “single best answer” (SBA).

A good example is:

Most cancers of the large bowel

A arise de novo in the mucosa

B arise from pre-existing adenomas

C have spread to the liver at the time of detection

D present first with low abdominal pain

E occur in the caecum

The correct answer is B.

This question simply tests recall of facts (knowledge).

Questions can be developed that test application of knowledge (clinical reasoning).

A good example is:

A 73 year-old woman complains of pain mainly in the limb girdles, associated with marked stiffness. Her symptoms are worse in the early hours of the morning and on waking. There is no abnormality on examination apart from mild generalised stiffness of the shoulder and hip joints. Your provisional diagnosis should most likely be confirmed by

A X-ray of the pelvis and shoulder girdle

B serum calcium and phosphorous levels

C serum alkaline phosphatase level

D erythrocyte sedimentation rate of 110 mm/h

E latex rheumatoid factor

The correct answer is D.

This question requires you to interpret the clinical material, make a diagnosis, and then select the most appropriate investigation. In this case the history suggests polymyalgia rheumatica as the most likely diagnosis, and hence an ESR of 110 is the test result most likely to confirm your diagnosis.

The AMC exam contains both types of questions and covers medicine, surgery, paediatrics, obstetrics and gynaecology, psychiatry and general practice. The standard is intended to be equivalent to final year medical student / intern level.

How should I study for the MCQ?

Many OTDs have spent several years pursuing a particular specialty, and therefore the MCQ tests many topics outside of their current area of practice. This can feel like quite a daunting prospect. If this is the case the best place to start is by doing a practice MCQ exam and assessing your performance in the different topics areas.

Focus most of your study on your weakest areas!

As the MCQ tests knowledge, you should focus on trying to acquire relevant knowledge. There is no substitute for reading. Choice of texts is a personal matter, but in general use the slimmer books designed for medical students rather than the massive tomes that are designed as reference texts. “Surgimed” in Collins Street, Hobart, is our local specialist book shop and usually stocks a complete range of texts.

You should assess your progress regularly by doing practice MCQs and adjusting your studying according to the results.

The AMC’s Anthology of Medical Conditions is recommended to help you work out the scope of the exam questions for both the MCQ and the OSCE.

The following MCQ books are recommended:

Australian Medical Council. Annotated Multiple Choice Questions, 2004. Blackwell Publishing.

Coales U & Khan S. Get through medical school: 1000 SBAs/BOFs and EMQs, 2006. The Royal Society of Medicine Press.

MCQs can also be accessed from the internet. Care needs to be taken to ensure that the site is reliable, the questions well written, and the answers are correct.

The following sites have MCQs of an appropriate quality. Note that some questions are aimed at specialist exams and will therefore be more difficult than the ones you will encounter in the AMC exam.

Beware! From time to time AMC MCQ “question papers” come on to the “black market” or are circulated by exam candidates. Steer clear of these papers. They are of dubious quality and may do more harm than good!

Basic hints on how to approach an MCQ exam

Read the whole question through carefully before you look at the answers. Make sure you understand the question. Some may have double negatives in them which may be confusing, and some may ask “which of the following is not…” (both of these are poor questions but may still be found from time to time).

Read through the selection of possible answers. If you know the correct answer…well its pretty straight forward! Often you will find that you’re not sure.

If you are not sure then use a process of elimination. In other words look down the list for answers that are definitely wrong. Eliminate as many wrong answers as you can. This way if you have to guess, you have reduced the number from which to guess and so increased your chances.

If you really don’t know, don’t guess at this stage, simply leave it blank and come back to it at the end.

Work methodically through the whole paper until you have finished. Don’t rush yourself, plenty of time is available.

When you have finished, go back to the beginning and find all those that you have left blank. You may now find that you can answer more of those that you didn’t know before, as some of the other questions that you have completed may have contained information that helps you.

Don’t be tempted to spend time going back and checking questions you have answered already unless you have a very good reason eg information in another question has made you doubt a previous answer. If you do this, you will become anxious, your performance will drop and you will probably start making more mistakes.

The next section provides some advice on what to do if you don’t know the answer to a question.

“Testwiseness”

This is how to increase your chances of answering the question correctly even if you don’t know the answer!

Like many things, exams are a game. A serious game admittedly, but a game with rules and strategies and loop holes none the less. Learning how to play the game can seriously increase your score.

As an exam candidate you have probably never thought about how to write MCQ questions! It is actually very difficult to produce well written questions (believe me, I’ve tried!). Candidates who understand how questions are written, and the difficulties in writing them, can exploit this knowledge by spotting those that are less well written and “give away” the right answer. Of course, a well written question will not “give away” the answer, but even in an exam of the standard of the AMC you will find some relatively poorly written questions.

Let’s look at some of the ways in which poorly written questions “give the game away”.

I have adapted this section from “Constructing Written Test Questions for the Basic and Clinical Sciences” by Susan Case and David Swanson, National Board of Medical Examiners.

Grammatical cues - These arise from the fact that question writers pay more attention to the correct answer than they do to the distractors. Thus if you see a grammatical error, it is likely that it is a distractor and not the correct answer. Consider the following question:

“A 60 year-old-man is brought to the emergency department by the police, who found him lying unconscious on the pavement. After ascertaining that he has a patent airway, the next step in management should be intravenous administration of:

A. examination of cerebrospinal fluid

B. glucose with vitamin B1

C. CT scan of the head

D. Phenytoin

E. Diazepam

Answers A and C are clearly wrong because they do not follow grammatically from the stem. This reduces your choice to B, D or E. The correct answer is B.

Use of absolute terms eg “always” and “never” – It is the nature of medicine that things are seldom absolute. An answer that is stated less absolutely is more likely to be correct than one that contains absolutes such as always or never.

Repeating a word or phrase – Sometimes a word (or its synonym) is used in the stem and then in one of the answers. This answer is likely to be correct.

The correct answer is the longest one – Exam writers are, quite appropriately, obsessed with making sure that the right answer really is right. To that end the correct answer may be the longest, most complete or most specific answer, whilst the distractors are shorter.

Implausible distractors - Another common “give away”, is the problem related to writing questions with plausible distractors. When examiners write questions they usually start with the idea, eg a clinical scenario or issue, next they determine the correct answer, and then try to think up some “wrong” answers (distractors) to act as other options. Writing good distractors is difficult. Of course, you can make questions intentionally easier to answer by making distractors more or less plausible.

Let’s look at a question from the AMCQ book. This has some implausible distractors and can be tackled by the process of elimination mentioned above.

Primary amenorrhoea is the USUAL feature in which of the following?

A Pregnancy

B Anorexia Nervosa

C Polycystic Ovarian Syndrome

D Turner syndrome (45, XO)

E Premature menopause

Your first response might be “gulp!” Now think about it. Primary amenorrhoea basically means “has never menstruated ever”. Well, if you’ve never menstruated, how could you get pregnant? Anorexia nervosa can cause secondary amenorrhoea as can polycystic ovarian syndrome. Premature menopause is completely implausible, and also causes secondary amenorrhoea, which leaves Turner syndrome as the only plausible option. It is possible to get this question right even if you don’t know anything about Turner syndrome!

Summary

• Do batches of MCQs regularly and assess your performance across all of the topics.

• Focus your studying on your weak areas.

• Spend just a little bit of time understanding MCQs and the concept of “Testwiseness” – it is of course no substitute for proper study, but might just turn a fail into pass.

انا حاولت اعرف معلومات اكتر عن طريقه امتحان AMC part I MCQs ولاقيت انه بصراحه مش صعب للدرجه اولا الامتحان عبارة عن single best answer (SBA). l والامتحان يشمل اسئلة من الباطنه والجراحة والاطفال والنسا والولاده ونفسية .

بالنسبه للكتب لاقيت انهم بيؤكدوا علي ثلاث كتب اساسيه وهم The AMC’s Anthology of Medical Conditions و كتاب Annotated Multiple Choice Questions, 2004 و كتاب Coales U & Khan S. Get through medical school: 1000 SBAs/BOFs and EMQs, 2006

وبيؤكدوا علي توخي الحذر من باقي المواقع علي الانترنت قبل التاكد من كيفيه وضع الاسئلة و ان الاجابات الموضوعه صحيحه فعلا .

أنا يا سيدي الفاضل ماخدتش تكليفي واين كان مسماك علي جواز السفر مش هيفرق مع الناس هنا ,لو جاي بيجوز سفر مكتوب عليه طالب في سابعه ابتدائي ولا هتفرق معاهم خلاص ,أنا حاولت أعمل التكليف في مستشفيات الشرطه لاني مكنتش متأكد إني هعرف اخلص من حوار الجيش و اسافر ,و بعدين ربنا وفق في موضوع الجيش ,اخدت تأجيل رحت مسافر بعد 10 أيام من الامتياز مخلص ,وجوازي كان عليه طالب بكليه الطب ,هنا استخدام جواز السفر كوثيقه لاثبات شخصيتك بس لكن متفرقش معاهم المعلومات اللي عليه ,أنا اللي الان معرفش إيه أخبار تكليفي ده اصلا ,وبعد سنتين خلصت ورق الطبيب الحر وغيرت الجواز لطبيب حر ,انصحك لو خلاص قررت تسافر متستلمش تكليفك وسيبها عايمه كده ,لحد متظبط امورك هنا وبعدها يحلها الحلال ,لو ملكش نصيب هنا يبقي ترجع تاخد امتيازك وأنت مرتاح

ملاحيظ بخصوص الكتاب الجديد

1- نوعيه الاساله متغيره الى حد كبير بقت اقرب الى cases منها للاساله القديمه واصعب نسبيا ومليانه تفاصيل مكانتش فى العادى بتبقي موجوده

مش عارف بقي هل ده معناه انه الامتحان هيبقي كده ولا ايه؟

2- الكتاب تقريبا ملون كله الكلام نفسه ملون وكميه صور تقريبا صفحه وصفحه حاجه تخنق بجد

3- حجمه متوسط لا حصل كبير ولا صغير علشان تنسخه لازم تقطعه ملازم ملازم

ان شاء الله لو كان فيه وقت هبقي افكرله فى فكره البتاع ده
